We ask participants to complete the following form.

FORM HAS RETURNED FOR THE NEXT SESSION–FALL 2017:

Is this your first RoCo Fit session?
(That is, something other than daily life activities, for example: bicycling, walking, running, swimming, aerobics, dance, sports, etc. are leisure time activities)
(Serving sizes: 1 cup of fruit or 100% fruit juice, or ½ cup of dried fruit; 1 cup of raw or cooked vegetables or vegetable juice, or 2 cups of raw leafy greens)
By Setting and sharing a goal, you increase your chances of success

Participants will be asked to submit a similar survey at the conclusion of the current season. There is a separate page for that survey.
